Squad Registration Form

(Issue 16 - 01st January 2012)

If completing your Squad Registration Form on
your PC then click File / Save As and save as a Word.doc with "YourTeamName.doc"
(ie: LymmSnrColts.doc) as the title.

If you have more than 30 players to register please create a second SquadReg Form and save as a Word.doc with "YourTeamName2.doc"
(ie: LymmSnrColts2.doc) as the title. 

Keep saved a copies on your PC for future additions and updates...

Squad Reg Forms should be submitted by eMail to squadreg@colts-rugby.org.uk

Alternatively these can be printed off or hand written in Black Ink and sent by FAX

Please Do Not send as a .JPG file and DO NOT re-format this form...
